Frequently asked questions

Answers to common questions about fees, visits, prescribing and privacy.

Do you take my insurance?

Arianna Perez is out of network, so we do not bill insurance, and that includes Medicare and Medicaid. Some plans pay part of the cost of out-of-network care, and your plan decides how much, if anything. How fees work has the details.

What is a superbill, and how do I use it?

A superbill is an itemized monthly receipt you can send to your plan to ask for reimbursement. It lists a diagnosis code, which your plan will see. Whether your plan pays is its decision, and we cannot promise that it will.

What is a Good Faith Estimate?

If you pay without insurance, you have the right to receive a Good Faith Estimate of what care will cost before you start. You can dispute a bill that is $400 or more above it. The estimate is not a contract. You can read more at cms.gov/nosurprises.

How much does it cost, and how do fees work?

Your fee is given in writing before your first visit, and we explain it before you book anything. An evaluation and a follow-up visit are charged differently. A limited number of reduced-fee places may be open, so ask when you write.

What happens at the first visit, and how often will I come back?

The first visit is an evaluation of about an hour: your history, your health and what you hope will change. Follow-up visits are shorter. Your clinician sets the pace with you, and we don't promise a number of visits.

Are you licensed where I live?

Arianna Perez is licensed in Oregon. Video visits are for clients who are in Oregon at the time of the visit. If you live elsewhere, we are glad to help you look for a prescriber licensed in your state.

What do I do in a crisis?

This office does not monitor calls or messages around the clock. If you are in crisis, call or text 988 any time. In an emergency, call 911.

What is confidential, and what are the limits?

What you tell us stays private and is shared only as the law allows. There are a few exceptions, such as a serious risk of harm to you or someone else, or a legal order. We go over them at the start of your first visit.

What is your cancellation policy?

Please give 24 hours' notice to cancel or move a visit. A later cancellation may be charged, up to the fee for the visit.

Do you prescribe controlled medication by video?

I make no promise about controlled medication. Whether to prescribe one is decided after an evaluation, under the federal and Oregon rules that apply at the time, and those rules can require a visit in person. The prescribing policy explains how decisions are made.

Will I need labs?

Sometimes. Some medicines call for blood tests before or while you take them. If you need one, I explain why, and you can have it drawn at a lab near you.

How soon can I be seen?

Openings change from week to week. Write or call and we will tell you the next time available. If you need help right away, call 988 or 911.

When you are ready

Write a few lines and we will reply by email or phone.
