Psychiatric care you can reach from home
Evaluations and medication management for adults across Oregon, by video or in our Portland office.

Meet Arianna Perez
I'm a psychiatric mental health nurse practitioner in Portland, Oregon. I see adults for a psychiatric evaluation and for ongoing medication management, in my office or by video from wherever you are in Oregon.
A visit is a conversation. I ask about your health, your history and what you hope will change, and we decide together whether medication belongs in the plan. I explain what a medicine can and cannot do, the risks, and what to watch for.
If you also see a therapist or a primary care clinician, I can work alongside them, with your permission.

How I practice
Time to talk
The first visit is long enough to tell your story, not just to adjust a dose.
Plain answers
I explain what a medicine is for, what it can do, and what to watch for.
Your decision
Medication is a choice we make together, and you can ask to change or stop at any visit.
Shared care
With your permission, I keep your therapist and primary care clinician in the loop.
How I can help
I see adults for a psychiatric evaluation and for ongoing medication management.
Psychiatric evaluation
An hour to go through your history, your health, your sleep and what you hope will change. You leave with a plan and a clear idea of what comes next.

Medication management
Shorter follow-up visits to see how a medicine is working for you, adjust it if needed and answer your questions, by video or in the office.

Starting a medication
Before a first prescription we talk through what it is for, how it may help, the common side effects and what to do if something worries you.

Changing or stopping a medication
Dose changes are planned together, one step at a time. Please don't stop a prescribed medicine on your own; call first.

Working with your care team
With your written permission, I share notes and plans with your therapist or primary care clinician so everyone works from the same plan.

How we begin
Starting care can feel like a lot. Here is the order of things.
- 01
Write or call
Send a message or call the office. We reply by email or phone, and we answer questions about fees before you book anything.
- 02
The evaluation
A first visit of about an hour, in the office or by video, to go through your history, your health and your goals.
- 03
Follow-up visits
Shorter visits to see how things are going. Your clinician sets the pace, and we don't promise a number of visits.
- 04
Regular review
From time to time we look at the whole plan together and space visits out when that makes sense.

Tyrese B.“Every question got a straight answer, including the ones I was embarrassed to ask.”
Hao L.“The office is quiet and the room feels warm. Nothing about the visit felt hurried.”
Darya S.“I knew what would happen at each step, and what each follow-up was for. That helped me relax.”
